Additional notes column with tags in bvl qc module

it would be nice to add an open text column (perhaps linked to bx feedback mod?) in teh data team helper.

As an example of why this would be ideal if I have 14 incomplete DSM forms, right now, I have to go in to each to find out why they are incomplete it would be nice to have a column with "awaiting Kelly Botteron to score to enter data" and be able to 'tag' the person.

Right now, as we try and close IBIS2, I am running into the situation where I go to check these massive lists and have to investigate why over and over so it would be super helpful to have a notes category.
